This article will walk you through creating a Supplier Evaluation Findings Form (Scorecard) where you will score the Supplier using a grading rubric with a predetermined set of criteria.

1. Creating the Findings Form
- Open the Supplier Evaluation Plan and scroll down to the Evaluation Findings Forms section. Click on 'Create Evaluation Findings Form' button
1.1. Click the forward Chevron to confirm.
- The Document Layout Selection should read as shown above. If not, hit the drop down arrow and select "Supplier Evaluation Checklist Layout" from the list.
2. Supplier Evaluation Findings Form
The Evaluation Checklist is created. You can specify at any time the current status of this by setting the Customer Status dropdown.
The document has automatically created links back to the originating profile and also to the originating Evaluation Plan.
At this stage you can, if required, select Co-Assessors who would have equal rights to edit and add content to this findings form
2.1. Recommendations and Summary
Based on the results of this particular findings form you can detail any recommendations or comments in these boxes
3. Completing the Form
Once all sections have been evaluated you can complete the findings form by clicking the following button in the toolbar. If you have not yet finished, simply click "Save" or "Save and Close"to save your work and finish at a later time.
When you click "Complete Evaluation Findings Form," both the Findings Form and the Evaluation Plan will automatically go to "Closed" status. The Evaluation Plan will then calculate the date for the next Evaluation and make a new version of the Plan for the future date. See the Process Flow Chart for Clarification
4. Additional Notes and Tips
4.1. Supplier Evaluation Plan: Manually Initiate Reevaluation
If required, you can manually initiate reevaulation of this supplier from the Supplier Evaluation Plan document.
